Community Coordinator: Positions: 2, Bangalore, Karnataka Qualification and Experience: • Master’s/Bachelor’s degree in social work, sociology, public health and relevant disciplines • Minimum of 3-4 years of progressively responsible professional experience in NGO sector specifically with private health care sector and vulnerable population • Self-motivated and able to motivate others • Commitment to public health values and TB control • Commitment to quality work by self, by team Roles and Responsibilities: Coordination with National TB Elimination Program – NTEP • Discuss implementation design and implementation plan with the STS / TU level and closely work with PHCs and/or assigned agencies • Participate actively in regular meetings with RNTCP-TU to share monthly progress of all the models and showcase KHPT’s contribution. Engagement with Community Structures (CSs) • Build capacity of Key Opinion Leaders (KOLs) and support CS for campaigns/events to promote positive messaging on TB related stigma and discrimination • Identification of Community Structures(CSs) and sensitize on TB and other health agenda • Develop link between CS and Patient Support Groups and NTEP – other stakeholders as per the project requirements • Organize monthly review meetings with Community Structures and motivate and support for TB control activities • Ensure and deliver capacity building / handholding for CS for active involvement and expected contributions from the CS. Piloting of Differentiated Care Model • Plan intensive in-person care and follow-up of DCM patients, and organize linkage and support activities for patients • Conduct monthly review meetings and address issues • Support needy patients and families with possible entitlements Formation and strengthening of Patient Support Groups • Establish Patient Support Groups at DMC level in coordination with RNTCP • Identify TB Advocates and build their capacity to advocate the TB patients • Identify TB patients with side-effects and link them to the medical facility Other related functions • Mapping, data collection, research/interviews, reporting and simple analyses • Documentation and reporting of all activities done • Work with the team for better planning, implementation and ensure deliverables are achieved on time • Conduct monthly review meetings and address issues • Work proactively with the line manager to assess the gaps and plan to fill the same Documentation & Reporting: • Document case-studies, good practices, reports and other documents as per the project requirements in preferred languages Salary: The salary for the Community Coordinator position is in the range of CTC of Rs. 15,000/- to 17,000/- per month which will be fixed based on qualification, relevant experience, and interview performance.
